Not every treatment divides in two

The split follows the rhythm of the treatment, not the flight calendar. Where a long wait is built into the work, there is a natural pause: after an implant is placed, the bone needs months to heal around it and nothing else happens in the meantime. A tooth reduced for a crown, by contrast, cannot wait months once the impression has been taken.

• Divides well: the healing months after an implant is placed, healing of an extraction site, the review that follows gum treatment.
• Divides badly: a crown on a reduced tooth, the gap between a bridge try-in and cementing it, a filling planned as a single visit.
• Depends on the case: tooth movement with clear aligners, covering a tooth after root canal treatment, making a night guard.

The records have to travel with you

The second dentist learns what happened in stage one only from what was written down. A phone photograph of a radiograph on a screen is rarely enough; the images are asked for as original files, with the date. If an intraoral scan was taken, the scan file belongs in the same folder. The written summary should show which tooth was treated, on what date, and what the next step is.

• The panoramic radiograph and, if one exists, the original file of the three-dimensional scan, with the date it was taken.
• A written summary: which tooth, what was done, on which day, and which steps of the plan are left.
• The name of the tooth numbering system used; under another system the same number points to a different tooth.
• Intraoral scans, photographs and the shade that was chosen.

The material has to be named in writing

This is where a second dentist most often gets stuck. Without the brand, diameter and connection size of the implant, the part that sits on top cannot be ordered, and finding it by trial and error takes a long time and does not always succeed. The same holds for crown material: zirconia, glass ceramic or something deliberately temporary, written down as exactly that.

A temporary is meant for weeks, not months

Temporary crowns and temporary fillings wear down, start to leak at the margin and let decay begin underneath. If one of them comes off, neighbouring teeth drift towards the space and the tooth in the opposing jaw drifts towards the space, so a new impression may be needed on return. The gum also takes its shape from the temporary, and after a long wait the final crown may no longer match it.

Who is in charge between the two stages

The question most often overlooked in split treatment is whose the middle period is. The first dentist is far away, the second has not taken the case on yet, and nobody is watching. So before departure it is written down who to turn to, how often things are checked and which symptoms cannot wait. Confirming in advance that the second dentist agrees to take the case on matters just as much.

A patient is not a courier between two dentists: the plan travels in writing, not in memory.

Why the two dentists should write to each other

When two dentists treat the same mouth in turn, what passes between them should not rest on what the patient remembers. The written plan, the answer to the second dentist's questions and the date both have agreed on belong in the file. Direct correspondence stops the same tooth being opened twice or the wrong component being ordered. Which language the correspondence runs in is settled at the start too.

If something goes wrong, where does it get sorted

A temporary coming off, a suture opening or healing that runs slower than expected are ordinary hiccups, and most of them are dealt with where you are. It helps to settle before departure who gives first aid locally and what would make an early return necessary. With facial swelling, fever, difficulty swallowing or a jaw that will not open, do not wait: seek dental or medical care the same day.
