Why teeth change colour over time

Teeth darken for two different reasons: stain that builds up on the surface, and thinning enamel that lets the darker dentine underneath show through. The two are not the same, and they do not lighten the same way.

Two separate routes to a change in colour

Teeth do not darken for a single reason. On one side there is colour that arrives from outside and settles on the surface: the stain left by tea, coffee and tobacco. On the other side there is the tooth's own colour, set by the enamel and the dentine beneath it together. Surface stain comes off; the tooth's own colour cannot be polished away. What you see in the mirror is usually the sum of both, and which one dominates decides what will actually help.

Stain that builds up on the surface

Enamel looks smooth, but its surface is not completely even. Coloured drinks, tobacco smoke and some foods cling to it. Where there is plaque or tartar, stain settles faster, because a rough surface holds colour better. This kind of discolouration shows first along the gum line, between the teeth and on the inner side of the lower front teeth. Brushing keeps it in check, and a professional clean removes most of it.

  • Tea, coffee, cola and dark fruit juices
  • Cigarettes and other tobacco products
  • Red wine, soy sauce and dark spices
  • Long-term use of certain mouthwashes

As enamel thins, the dentine shows through

Enamel is semi-translucent and lets some light pass through it. The dentine underneath is naturally yellowish. As the enamel thins, dentine takes over the colour of the tooth and the tooth looks yellower. Two things thin it: wear and acid. Brushing with stiff bristles, heavy pressure and wide scrubbing strokes takes enamel away over the years. Acidic drinks, frequent reflux and vomiting dissolve it. Here there is no stain on the tooth at all, only a missing layer.

When every tooth yellows together, that is usually the work of years; when one tooth is darker than its neighbours, something else needs looking at.

Dentine thickens with age

Dentine keeps being made throughout life. Over the years the chamber inside the tooth narrows, the dentine layer grows thicker and its colour deepens. Enamel has been wearing down across the same span. Put together, teeth look darker at sixty than they did at thirty. This is ordinary change rather than disease, though it shows up earlier in people who brush heavily.

When a single tooth darkens

If one tooth is clearly darker than the ones beside it, something has happened inside that tooth. A knock is the usual cause: a fall, a collision, a blow during sport. The blood vessels inside the tooth are injured, and the blood products left behind stain the dentine grey, brown or pinkish. The change can appear weeks, sometimes years, after the injury. Teeth that have had root canal treatment also darken over time. Gel applied from the outside does little here, because the colour sits inside the tooth.

Discolouration caused by medicines

Some medicines change tooth colour from the inside. While teeth are still being formed in the jaw, that is during pregnancy and in the first years of life, certain antibiotics are laid down within the dentine; the grey or brown bands they leave become visible only once the teeth come through, and brushing does nothing to them. Some mouthwashes work the other way round, staining from outside: used for long stretches they leave a brown film on teeth and tongue, which a clean removes.

What whitening changes, and what it does not

The gel passes through enamel and breaks down the colour molecules in the dentine, so it acts on the tooth's own colour. Surface stain is a different job, and a clean comes first there. There are also places whitening cannot reach. Results vary from person to person, the same gel does not lighten every mouth to the same degree, and the shade drifts back a little over the months that follow.

  • Fillings, crowns and veneers do not lighten; as the teeth around them get lighter, the mismatch becomes more obvious.
  • A tooth darkened after a knock will not respond to gel applied on the outside, and a different route is needed there.
  • White patches on enamel do not disappear with whitening and can stand out more clearly afterwards.
  • Root surface exposed by receding gums does not lighten with whitening.

Does a yellow tooth mean an unhealthy tooth?

No. Natural tooth colour varies from person to person, and a healthy tooth with thicker dentine simply looks yellower. Shade on its own says nothing about decay or gum disease; what matters is the state of the gums and of the tooth itself.

Does whitening damage enamel?

Whitening carried out under dental supervision does not change the structure of the enamel. Sensitivity lasting a few days during and after treatment is common and usually settles on its own. Very strong products used without supervision can burn the gums.

Do whitening toothpastes lighten the colour?

They reduce surface stain with fine abrasive particles and a few added ingredients, while the tooth's own colour stays as it is. Used hard over a long period they wear enamel down as well, and the tooth then looks darker.

Will a darkened tooth go back to its old colour?

The pinkish shade seen in the first weeks after a knock sometimes fades. A tooth that turns grey or brown mostly stays that way, and the tissue inside it needs checking. An x-ray and a test of how the tooth responds to cold show where things stand.

Will my fillings and crowns get whiter too?

No. The gel acts only on tooth tissue. As the surrounding teeth lighten, older fillings and crowns look darker beside them, which is why fillings in the visible zone are usually replaced a few weeks after whitening.

Does tooth colour improve after stopping smoking?

New stain stops building up, so the teeth darken far more slowly. Tar that has already settled does not come off by itself and needs a professional clean. The gain for the gums is larger than the change in shade.
