A permanent tooth completely knocked out by trauma is one of the genuine emergencies in dentistry. The chance of success depends directly on time and on how the tooth is stored. What matters is keeping the periodontal ligament cells on the root surface alive; once they dry out, the tooth cannot reattach.

What to do

  • Handle the tooth only by the crown, the white part normally visible in the mouth
  • If dirty, rinse gently for a few seconds in cold milk or saline
  • If possible, place the tooth back in its socket and hold it in place by biting on a clean cloth or gauze
  • If it cannot be replaced, store it in milk; if there is no milk, the person's own saliva can be used
  • Reach a dentist without losing time

Why milk works

The osmotic pressure and pH of milk are close to those of cells, so it can preserve ligament cells for hours. Saline is also suitable. Water is the worst option; it destroys the cells quickly.

How much time matters

If the tooth is replanted within the first thirty minutes, the chance of reattachment is markedly higher. Storing it correctly extends that window, but the longer it stays dry the lower the success. How the first response is handled therefore shapes how well the later treatment works.

This is one of the rare situations where knowledge alone saves a tooth. Having one person present who knows what to do changes the outcome.

What if a baby tooth is knocked out?

A baby tooth is not replanted, because of the risk of damaging the developing permanent tooth beneath. The child should still be assessed by a dentist.

Should I keep the fragment if the tooth breaks?

Yes. In some cases the fragment can be bonded back on. Store it the same way, in milk or saline.
